Assumption of Risk

I understand and acknowledge that participation in martial arts training at Roanoke Kung Fu involves physical activity that includes but is not limited to striking, grappling, self-defense practice, partner drills, conditioning, and sparring. I understand that such activities carry inherent risks of injury, including but not limited to bruises, sprains, broken bones, head injuries, paralysis, or even death.

I voluntarily choose to participate in these activities, fully aware of the risks involved, and I accept full responsibility for any injury, loss, or damage that I may sustain.

Release of Liability

In consideration of being allowed to participate at Roanoke Kung Fu, I hereby release, discharge, and hold harmless Roanoke Kung Fu, its instructors, employees, volunteers, and agents from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury I may sustain while participating in training, whether caused by negligence or otherwise.

Medical Authorization

I affirm that I am physically fit to participate in martial arts training and have no medical condition that would prevent safe participation. In the event of an emergency, I authorize Roanoke Kung Fu to obtain medical treatment on my behalf. I understand that I am responsible for all medical expenses incurred.

Photo/Video Release

I grant permission for Roanoke Kung Fu to take and use photos or video recordings of me during training or events for educational and promotional purposes.

Membership Agreement

I agree to follow the rules, etiquette, and safety guidelines of Roanoke Kung Fu. I understand that failure to do so may result in suspension or termination of my membership without refund.

I acknowledge that this waiver is aone-time, continuing release and will remain in effect for all current and future participation at Roanoke Kung Fu, unless revoked in writing.

Acknowledgment of Understanding

I have carefully read this Waiver and Release of Liability, fully understand its contents, and voluntarily agree to its terms. I am aware that by signing this document, I may be waiving certain legal rights.
